We are currently accepting new patients on our list - if you live in our practice catchment area and wish to register with us, please complete a GMS-1 family doctor's registration form.

Download, print complete and return a spearate form for each patient wanting to register. Bring the completed forms in to the surgery with a copy of the following documentation:

  • One photographic proof of identification, either a photo driving licence or passport
  • And one proof of your current address, which can either be a recent utility bill (no less than 2 months old), or a mortgage / bank statement or a letter from your landlord or a rental agreement.

We can arrange for our new patients to have a new patient health check with our practice nurse to check that all required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Please note that not all required information is captured electronically. This may necessitate further calls to the practice before the registration process is complete.

Temporary patient registration

If you are ill while away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Change of personal details

Please contact us if you need to update your personal details e.g you change your name, address, telephone number, or email address. The reception team will be happy to assist you in getting the correct details added to our systems.
